Types of Membership
- Individual Member: Anyone who agrees with the association's purpose and has graduated from a medical school (including dental and traditional Chinese medicine programs) recognized in Taiwan or abroad, and holds a physician's license issued by Taiwan's health authorities, may apply for individual membership by submitting a completed application form. Membership is granted upon approval by the Board of Directors and payment of fees. The application fee is NT$1,000, payable upon joining, and the annual membership fee is NT$500.
- Professional Member: Individuals who meet any of the following qualifications may become professional members upon recommendation by two current members, approval by the Board of Directors, and payment of fees. The application fee is NT$600, payable upon joining, and the annual membership fee is NT$300:
- Holders of medical personnel licenses in Taiwan.
- Individuals engaged in lifestyle adjustments and health promotion-related work.
- Sponsor Member: Individuals or organizations that agree with the association's purpose and provide substantial funding or resources may apply for sponsor membership by submitting a completed application form. This will be approved by the Board of Directors.
- Honorary Member: Individuals who have made significant contributions to promoting lifestyle medicine may be awarded honorary membership upon approval by the Board of Directors.

How to Apply for Membership
1. Complete the application form and sign or stamp it, then send it via email or mail to the Taiwan Lifestyle Medicine Association to complete the application process.
